Regent Medical Solutions are physician’s advocates who first began working with early adopters of Electronic Health Records (EHR) Systems over 15 years ago. Much of our initial work was for the Quality Improvement Organization (QIO), in support of the Centers for Medicare & Medicaid Services (CMS). The results from those first projects would eventually be compiled into what is now commonly known as the criteria for Meaningful Use. Throughout this time span, we have been educating physicians and countless healthcare support professionals in their transformation from paper to electronic heath records.  Most recently, Regent consultants assisted Ohio Regional Extension Center providers in EHR selection and implementation, followed by successful attestation to Stage I of Meaningful Use.

Stage 1 Meaningful Use

With all of the buzz surrounding the CMS’s proposed Meaningful Use Stage 2 requirements and the ICD-10 deadline extension, it is easy to forget that the clock is ticking on the Stage 1 Meaningful Use requirements. Physicians who want to qualify for the full, five-years’ worth of meaningful use incentives ($44,000 total) must register, adopt a certified EHR system and submit 90-days’ worth of data by Jan. 1, 2013. That means, in a perfect world, your practice must have incorporated your EHR system into its workflow and be collecting the needed data sets by Oct. 3, 2012.

We have reached the tipping point in terms of EHR adoption. Penalties for failing to e-prescribe began in 2012, and in 2014 and 2015, physicians will face mounting financial penalties from Medicare and Medicaid if they don’t adopt an EHR. Commercial payers are also adopting various kinds of accountable care programs that require submission of clinical data.

Meaningful Use Compliance – Stage 1

Your first year of participation in the EHR Incentive Program and thus your first incentive payment involves a 90 day reporting period.  To attest for the Medicare EHR Incentive Program in subsequent years, and thus receive your second incentive payment, you will need to have met Stage 1 meaningful use for a full year.  There seems to be moderate confusion regarding what triggers the Stage 1 Meaningful Use incentive payments and the ability to begin Stage 2.  To help clarify:

Stage 1 ( Year One ) :  90 day reporting period + successful attestation  =  18k payment

** ( If attestation is completed by Jan 1 2013 )

Stage 1 ( Year Two ) :  365 day reporting period + successful attestation  =  12k payment

Stage 2:  Cannot begin until successful attestation of Stage 1 ( Year Two )  has been completed.

For many practices, this 365 day reporting period may prove to be a daunting task.  Regent is there to help.

Stage 2 Meaningful Use

Stage 2 will be upon us soon. And a full year of operational use of capabilities will be required (rather than the 3 months for the first year of Stage 1). Waiting until the final rule is issued to start moving is poor strategy for success. Even though the rules are not final, many are unlikely to change and some are indeed set in stone. An example of this is the move of most Stage 1 menu options to core.

Based upon the Stage 1 feedback from CMS, AMA, MGMA and others, it is clear that now is the time for organizations to work in earnest to prepare for the increased expectations of Stage 2. As soon as the quality requirements are final, both hospitals and EPs need to focus on capturing and electronically reporting on CQM..
